The entire album is loosely based on the concept of killing yourself but the inner parts of yourself. In this song the lyrical content pertains to killing yourself but watching yourself die in a third person view and seeing the effects caused by committing suicide only to be on the edge of returning to life but are unfortunately dead hence "just tell me that this isn't real"
